Bremelanotide (PT-141) is a peptide that acts as a non-selective agonist of melanocortin receptors, which control sexual function/arousal, appetite, immunity, and other physiologic actions. While it was originally studied as a sunless tanning agent, researchers quickly noticed that males achieved spontaneous erections and increased rates of ejaculation. Bremelanotide is currently only FDA-approved for treatment of Hypoactive Sexual Desire Disorder (low sexual desire) in premenopausal women, however, many studies have documented improved sexual outcomes in men as well. Stimulation of melanocortin receptors produces different mechanisms of action in males, increases nitric oxide in the penis leading to vasodilation and erection, compared to females, stimulates dopamine release in brain areas responsible for sexual motivation, arousal, and desire. Unlike phosphodiesterase inhibitors which act directly on vascular structures, bremelanotide increases neurotransmitter signaling to indirectly stimulate genital blood flow while increasing sexual desire responses in the brain. Injectable bremelanotide is the most effective route of administration with nearly 100% bioavailability, however, nausea has been reported ~25% of patients and administration is limited to 8 doses/month due to skin hyperpigmentation. Intranasal application is more convenient, allows for more frequent use, and has limited skin hyperpigmentation and nausea. Nasals sprays have been reported to provide similar efficacy compared to injections in increasing arousal, desire, genital response to stimulation, and intensity and rates of satisfaction in men and women. Emerging studies have found that bremelanotide has a positive synergistic effect with other medications, like tadalafil, to enhance the erectile response to PDE5-inhibitors and successfully improve sexual function when patients who do not respond to other medications.
 
Tadalafil is a PDE5-inhibitor that dilates blood vessels to increase blood flow to the penis and treats erectile dysfunction in men. It is typically administered as an oral pill, however, recent studies have suggested that intranasal sprays may allow for better absorption. However, an increasing number of women are reporting improved sexual response, sensation, and satisfaction with tadalafil. Women do not typically rely on genital blood flow as much as men, but rather lubrication from sexual excitement. Likewise, tadalafil does not work in men unless they are sexually excited. In combination with bremelanotide, the effects of tadalafil are amplified and ensure you are roaring to go when the time comes (pun intended). 
 
Oxytocin is a peptide hormone produced in the hypothalamus of both men and women. It is commonly called the "love hormone" or "cuddle drug" due to its roles in reward responses, decreased anxiety, social bonding, maternal behaviors, romance, arousal, sexual activity and reproduction, and orgasm and sexual satisfaction. Oxytocin increases drastically during sexual activity in both males and females, and is believed to be responsible for emotional bonding with sexual partners. It is also thought to decrease auditory noise from the environment to allow for better perception of emotional and social cues, which may improve feelings of connection and intimacy. The production and secretion of oxytocin is controlled via a positive feedback mechanism, meaning initial release stimulates further production and release. Oxytocin may provide synergistic effects to the Libido Enhancing Nasal Spray and stimulate natural oxytocin production, leading to deeper connection, intimacy, arousal, satisfaction, and orgasm. 
 
Gonadorelin is a gonadotropin-releasing hormone (GnRH) agonist that induces production and secretion of gonadotropins, follicle-stimulating hormone (FSH) and luteinizing hormone (LH), by the pituitary gland. LH and FSH stimulate gonadal production of sex hormones and are responsible for development, growth, sexual maturation, and proper reproductive function in males and females. Due to its early mechanism of action in this pathway, a deficiency of GnRH will also impair downstream pathways, causing significant sexual dysfunction and loss of libido. Gonadorelin is used for the treatment of primary hypothalamic amenorrhea, hypogonadotropic hypogonadism, delayed puberty, cryptorchidism, infertility, and impotence. Studies have found that administration of gonadorelin increases sexual behaviors in women, and improves sperm concentration and libido in men. Intranasal gonadorelin has also been reported to significantly reduce depression and anxiety in women with severe premenstrual syndrome and men with sexual dysfunction.
